License Agreement

I give The Meadows Racetrack & Casino and its representatives, permission to obtain and verify credit information from any Source and to request information from any recognized credit bureau. I will not hold any of these entities or The Meadows Racetrack & Casino responsible or liable for the information released in the ordinary course of business. I understand there may be a cost associated with my bank and agree to pay all fees to obtain the credit information needed and The Meadows Racetrack & Casino will not be held responsible for any fees.

Before drawing on my line of credit, if granted, I agree to sign the counter check in the amount of the draw. I authorize The Meadows Racetrack & Casino to complete any of the following missing items on the counter check: the name of payee; any missing amounts; a date; the name, account number, address and/or branch of any bank or financial institution; and any electronic encoding of the items.

I acknowledge that I have the ability and intent to legally repay any advance of money to The Meadows Racetrack & Casino.

I agree that each draw against my credit line is separate advance of money by The Meadows Racetrack & Casino. If I receive the advance before I execute a counter check, I promptly will sign a counter check in the amount of the advance.

I agree that Pennsylvania law exclusively governs the terms of the credit line, advances or counter checks. I agree that The Meadows Racetrack & Casino may litigate any dispute involving the credit line, the debt, or the payee in any court, state or federal, in Pennsylvania. I submit to the jurisdiction of any court, state or federal, in Pennsylvania.

Besides any amounts authorized by law, I will pay all costs of collection, including attorney's fees and court costs.

THE MEADOWS RACETRACK & CASINO ENDORSES RESPONSIBLE GAMING: You may suspend your credit privileges at all Casinos ("licensed facilities") in Pennsylvania by submitting, in person, a completed Request for Voluntary Credit Suspension form to The Meadows Racetrack & Casino’s credit department or other certified holder's ("licensed facility") credit department or to the Board. A submissions to the Board may be made at the Board's office at a license facility, at the Board’s Harrisburg office or one of the Board's regional offices.

I agree that the information set forth above is true and accurate to the best of my knowledge.

"I certify that I have read and understand this application and its terms and I execute this document voluntarily and with full knowledge of its significance. I authorize The Meadows Racetrack & Casino to conduct any investigations necessary for the approval of my credit limit. I am aware that this application is required by the regulations of the Pennsylvania Gaming Control Board. I understand that a Counter Check issued by The Meadows Racetrack & Casino is identical to a personal check and may be deposited in or presented for payment to my bank or other financial institution. I acknowledge that willfully drawing or passing a credit instrument with the intent to defraud, including knowing there are insufficient funds in my account is a crime in the Commonwealth of Pennsylvania that may result in criminal prosecution. I am also aware that providing false or misleading statements or omitting information on this application may subject me to civil or criminal penalties."

Payment Disposition for each individual Counter Check: Less than $5,000 will be deposited not more than 15 days; $5,000 or more will be deposited not more than 30 days.

("NRT") and its affiliates to obtain consumer reports, to contact financial institutions, and to check my consumer credit, employment, bank and gaming history in order to evaluate my credit application as well as to update and/or review my account to provide services requested by me, as necessary or as required by law. I authorize Hollywood Casino at The Meadows, NRT or its affiliates to verify such information through any source and to report any information to another casino, excluding credit report data obtained from a credit agency, and to exchange any information with any of the affiliates of the Company. I hereby release and waive, and agree not to bring at any time in the future, any claims or demands against NRT or its affiliates relating to any credit investigation made pursuant to my authorization and instructions herein, including, without limitation, the collection, processing, and transmission of my information and data.
