I, the undersigned, understand and agree that this is a Front Money Application and Agreement (this "Agreement") between me, Sky Warrior Bahamas Limited d/b/a Baha Mar Casino ("ASWBL"), and Sky Warrior US LLC ("SWUL," and together with SWBL, "Sky Warrior"). I agree that all of the terms of the Credit Application, Authorization and Credit Agreement between me, Sky Warrior Bahamas Limited, and Sky Warrior US LLC (the "Credit Agreement") shall apply to this Agreement, and in the event of a conflict, the terms of the Credit Agreement shall govern. I affirm that on this date I have received cash and/or chips ("Front Money") from Sky Warrior (the providing of the Front Money being the "Casino Credit Services") for the above amount and I unconditionally promise to repay such amount, in accordance with the terms established by SWBL. I represent and warrant that I have the above amount on deposit in the disclosed financial institution to which I am an authorized signatory. These funds are free from claims and are subject to the Negotiable Instrument(s) that I will sign following the completion and approval of this Agreement. I authorize Sky Warrior, its affiliates, and their representatives and agents to micro-encode, deposit manually, electronically, including electronic funds transfer (EFT), ACH debit or bank draft, or by any other lawful means, against any or all financial institutional account I hold or to which I am a lawful signatory or process as a check transaction against same.

I hereby expressly consent to the entering into and enforceability of my electronic signature to bind me to this Agreement. I understand that I have the right to execute this Agreement in non-electric form. To obtain an electronic or paper copy of the electronic signature and Agreement, please send an email to credit@bahamar.com requesting the same. This consent can be withdrawn by submission of a written, specific request to guest services at credit@bahamar.com. Withdrawal shall be effective for the period commencing on the date the request is received by Sky Warrior.

I, the undersigned, hereby authorize and instruct Sky Warrior, its agent VisuaLimits, LLC ("VL") and their respective affiliates, as is necessary or required by law, to aggregate report data in order to evaluate my Front Money application included in this Agreement as well as to update and/or review my account to determine whether SWBL will provide the Casino Credit Services requested by me. I authorize Sky Warrior, VL and their affiliates to (a) verify such information through any source, (b) report any information to another casino, and (c) exchange any information with any of the affiliates of Sky Warrior and VL. I hereby release and waive, and agree not to bring at any time in the future, any claims or demands against Sky Warrior, VL and their respective affiliates relating to any investigation made pursuant to my authorization and instructions herein, including, without limitation, the collection, processing, and transmission of my information and data. In the event of a conflict in any of the terms contained in this Agreement, any Negotiable Instrument, or any other casino related document between me and Sky Warrior (collectively, "Casino Agreements"), the terms of this Agreement shall govern.
